A devastated mum is pleading for answers after her son died from injuries sustained during a workplace "explosion". Fabian Rozniatowski, 19, from Littleover, Derby, tragically died on his mother's birthday, five months after the freak accident happened in January 2023. Agnieszka Rozniatowska said her "world fell apart" following the passing of her son.

Fabian was working at a military pyrotechnics factory run by PW Defence in Draycott, when the accident took place. In November that year, an inquest was opened at Chesterfield Coroner's Court. The court heard that the teen was working in a room alone mixing compounds to be used in smoke grenades when the "explosion" happened at around 9pm.

A supervisor said he was “heavily burned and calling out for help” straight away. He was treated in the site’s medical room before medics attended. The inquest was adjourned pending the outcome of an investigation which is being led by Derbyshire Police. The Health and Safety Executive is providing assistance to the investigation. "It's two years and I cannot find out what has happened," said Agnieszka.

Speaking through an interpreter, she said: "On May 11 (2023), my world fell apart. That was my only child. I was very proud of him. He was always playing football and sport was very important to him. Now it's just sadness and sorrow. What was the cause of the accident? If I ring up to ask what is the latest from HSE, I just get told it is still being investigated and in progress. I want to know happened to him. It is very painful. I lost my dreams and my future.".
